The Vital Statistics office is located at the Historic Courthouse in downtown Cleburne 2 N. Main Rm 128.

A birth record is closed until the 75th anniversary of the date of birth as shown on the record filed with the local registrar.

You must be a qualified applicant in order to apply for a birth certificate (a parent, grandparent, sibling or yourself). Any one else must have a signed (original) statement authorizing them (naming them) to pick up the birth certificate. This statement must include a copy of your state/government issued ID. The person named in your request must present their original state/government issued ID also.

The following information is required:

  • Full name as it appears on birth record
  • Date and county of birth
  • Parents' names, including mother's maiden name
  • Your relationship to person named on certificate
  • Your valid mailing address
  • Purpose of record
  • In writing you will need to send a copy of your valid state issued ID
  • In person you will need to provide your original valid state issued ID
  • Cost per certificate is $23.00, payable to Johnson County 
  • Please do not send cash through the mail, we accept business checks, money orders, or cashiers check

The same information is required if requesting a birth certificate by mail.

 All fees are due and payable at time of service.

•Personal checks are not accepted.

•Business checks are accepted with company information on check.

•Credit cards are accepted with a small fee.
